With all respect to Dr. Noel this is not a lateral strabismus. What we're seeing is a bilateral sixth nerve paresis. The unopposed medial rectus muscle causes her to develop an inward turn ( esotropia)
When Hillary tries to look to the right she has weakness of the the right lateral rectus muscle and the left eye turns in excessively. This is caused by Herring's law of equal innervation. This is not typical of Parkinson's disease where problems with vertical movements are very common and later horizontally. The eyes tend to freeze in straight ahead position late in that disease. There are photos of both eyes turning inwards at the same time which only occur with a bilateral sixth cranial nerve weakness. The most likely cause of her presentation is increased intracranial pressure as the lateral rectus muscle that pulls the eye outward is the most commonly affected muscle due to the long course of the sixth cranial nerve from it's origen in the brainstem to the lateral rectus muscle. The increase in her intracranial pressure could possibly be from a recurrence of her venous sinus thrombosis or some other cause. It also explains her difficulty going up the stairs. Of course when the eyes are misaligned, diplopia or double vision is experienced immediately which would make it difficult to ascend the steps. In the past she used Fresnel prisms for this very reason of double vision. Expect some excuse for the debate to be postponed. Unfortunately for her and her family she is very ill.?
